Thanks for attaching a copy of the questionnaire file to your request. I saw that your movie file is a Quicktime format. Even if it does play on your computer, I wonder if changing the video file format to one like .mpg, .avi, .asf, .wmv, .dat, or .mpv that is automatically more compatible with Windows Media Player might fix the problem. Would you be able to give that a try?
