HTML/Javascript errors when using custom items (case 3294)

I am banking my head against the wall with a questionnaire that I had working perfectly fine until I needed to add in an option to skip each question. I have multiple responses per page, each question has multiple options selected using radio buttons. I also found some Java Script to check to make sure each question was answered before submitting. All was working well until I added in an option to skip each question. Now I am getting random HTML syntax errors, code 0, only when I open my .html doc in various editors and in Firefox's debugging feature, there is absolutely nothing wrong. Any suggestions in how to debug?

FYI the survey has a total of 6 html docs and this problem is occurring on 5 but the one without issues is exactly like the rest so I am stumped.

    Thanks

    Louise
