Data stored under wrong variable names

Hi, we've been using MediaLab in our lab for the past three years and haven't had any problems with data being written in three experiments. However, this last time after I added a few more variables to the experiment, it seems that the data is being shifted so that the responses are being recorded under the incorrect variables. I have tried clearing out the data and running the experiment several times (always starting with a clean data file), and I have checked the questionnaire files many many times to make sure that the ordering is correct and can't find anything wrong. Any ideas what might be going wrong? I'm providing one of the questionnaire files.

    Thanks for your help!
