Examples of HTM codes

Can someone please post an example of the exact HTM coding for a variety of popular question types? I have downloaded several .htm files (for example, demographics.htm, which was provided by Jarvis), but it does not include the coding that allowed someone to create that HTM file.

I would love to just see some examples of the .txt files (or whatever) that have the exact coding info used to generate an HTM file used for custom items.

IN PARTICULAR: I would like to have several scale response questions on one page (vs having Ss scroll from one question to the next, page after page after page). This is such a basic fundamental thing: does anybody have any examples of the coding that will allow me to create this?
