Data recording problems

I have a custom item that is recording the correct data, but the data is preceded with the variable name. For instance, for the first question on the motivation item, the data is being recorded as motivation_01=2, where 2 is the response. The html looks exactly like the other items' html which are all recording data correctly. Anyone know why this could be happening?

Another issue is that I have a custom item with only 1 question. There are 10 possible choices, so there are 10 possible values. I have it set so that the participant can only choose 1 answer. However, the data is being recorded as "ok." Again, the html looks exactly like the other items' html that are working correctly. Any suggestions here?
