Multiple fill-in-the-blank item

Hi there,
I am wondering if anyone has a custom item with has 2 fill-in-the-blank boxes. I basically need the participants to be able to enter 2 numbers on the same screen, and make sure that these numbers add up to 10.

E.g.

I would keep ___ dollars
I would leave ___ dollars
