Custom item with various html form elements

Hi, I was struggling to create a page contains three different html form elements: text input, radio button, and drop down menu.

I named these three elements individually as xxx_o1, xxx_02, and xxx_03, and add a custom item named xxx. But somehow MediaLab couldn't correctly show the html page (actually, it just gave me a blank page). If I seperate these three form elements, for example, using only the radio button, then this item can be correctly presented.

Please give me some suggestions. I really want these three questions appeared at ONE page. Thank you.
