This requires a custom item because MediaLab typically presents 1 item at a time. You can check out the Custom Items section of the user's guide to get a feel for how it works. There are also posted samples around on these forums. I could also help point you in a direction if you would like to describe the scales you'd like to use. E.g., 1 to 7 Likert. Continuous slider from 0 to 100, etc.

I couldn't find any relevant samples, where should I look for? In the questionnaire that I am trying to set up, I want to display a profile with a text description, followed by 6 independent questions appearing at the same time. Each question consists of 5 Likert-Type scales. Thank you.

Hi, I'm having the same issue where I would like to have a scenario at the top of the page and then five questions (2 fill in the blank, 3 Likert) relating to it below. Blair, I tried opening the sample que file you sent the previous member but it gave me an error message saying it was in read only format. Is there a simple way you can explain the steps we need to take to place multiple questions on a page? Thank you.

Have you checked out the users guide--i.e., the section on creating custom items? There is a step by step description there but I'd be happy to elaborate on anything that's not clear. There are also samples in the MediaLab samples folder that accompany the users guide section on the topic. I believe you can also find some multiple likert samples in the shared materials forum.
Thank you for sharing this. When I run it in MediaLab, I just get a blank screen, where the htm file should be displayed, and a "Continue" button at the bottom. I currently have MediaLab version 2004 - is it because of the older version or is there another problem?
