help creating a custom item

How can I create a page of my survey with two or more different questions on it for subjects to answer, particularly when they are different types such as fill-in combined with scale responses?

I have the same problem and would really appreciate any help possible:

I need to create a question like this:

1. The anticipated graduation date is:
a. May/Summer 2010
b. December 2010
c. May/Summer 2011
d. December 2011
Other (please specify)_________________

So, the last will be a fill-in-the-blank and the others are scale items and I really dont know how to do it.

However for this purpose, you might simply get by using the <specify> function in your questionnaire. e.g., in the case above, you would enter this option in your scale response:

Other (please specify) <specify>
